A concern has a context
The first account of a concern matters. So do the relevant observations, incidents and decisions that surround it. The ResiNotes Safeguarding Centre includes evidence links so authorised reviewers can connect a safeguarding concern with relevant chronology, risk, incident and missing-from-care records.
Keep the source visible
Linking evidence helps a reviewer follow where information came from. It also supports a more coherent account than repeatedly copying fragments between forms. A link is a connection to consider; it does not establish that two events have the same cause or remove the need to evaluate the information.
Bring professional involvement into view
The centre provides routes for recording professional actions, referrals and strategy work. Keeping those records alongside the safeguarding picture helps authorised staff follow the people involved and the work that has taken place. Access varies with the user’s role and permissions.
Record the decision and return to it
A management review can record the current position, update the concern’s status, publish appropriate guidance and set a next review date. The useful outcome is a record that explains the decision and what should happen next. Professional judgement remains essential: connected records support a review, but they cannot determine its conclusions.
