A calmer route through health and additional needs
Health information is easier to use when it is organised around the young person’s needs, not scattered across one long page or hidden behind disconnected records.
Start with what is known and what is still developing
Additional needs may be diagnosed, suspected, under assessment or described through ongoing support. A useful record should allow those stages to be distinguished without forcing staff to label a child before the evidence is clear.
Connect the supporting work
Appointments, assessments, documents, professional feedback and the young person’s views should be linkable to the relevant area. That makes the developing picture easier to understand while keeping each source record intact.
Use panels to reduce the load
Clear sections can separate identity, diagnosis or assessment, support needs, appointments, evidence and review. The result is easier to navigate on a phone and less likely to make important information disappear inside a long form.
Good organisation does not reduce the detail. It makes the detail usable.
Keep the child’s experience visible
Health support is not only a list of appointments. The young person’s wishes, communication and experience should remain visible alongside professional information and practical actions.
